Immunization Office
The Immunization Office is located in Room 114 of the Student Support Services Building (SU-80) south of the University Center on the Boca campus.
Immunization Policy
To comply with Florida Administrative Code 6C-6.001(5), Florida Statute 1006.69 and the Florida Atlantic University Immunization Policy, all students, prior to registration, must submit a FAU Immunization Form. All students born after 1956 must also submit documented proof of immunity to Measles and Rubella. All students living on campus are required to submit documentation of vaccination against Hepatitis B and Meningococcal Meningitis or sign a waiver (see Immunization Form, section C) for each vaccine.

The FAU Immunization Form is available for download and printing in PDF Format only. In order to view and print this form, you need to have the Adobe Acrobat Reader installed on your system. You may download the freely available Acrobat Reader software from Adobe Systems Inc. Versions of Acrobat reader run on DOS, Windows, Macintosh and Unix systems. The form consists of four pages and should be read carefully. All students must complete Section A and sign in Section C. Students born after 1956 must complete Section B, Part 1 if official immunization documents are not attached to the form. Students living on campus must complete Section B, Part 2. In addition, students under the age of 18 must have a parent/guardian sign in Section C.
How To Submit the Immunization Form
The completed form should be mailed to Florida Atlantic University, Student Health Services Immunization Office, 777 Glades Road, SU80, Room 114, Boca Raton, FL 33431 or faxed to 561-297-2769. If you have any questions, phone 561-297-0048.
Exemptions
- Temporary medical exemptions - must be submitted by the attending physician and include a reason for the exemption.
- Religious exemptions - contact FAU Immunization Office for more information.
- In the event of a Measles/Rubella emergency, exempted students will be excluded from all campus activities, until such time as is specified by the County Health Unit.
