2007-08 STUDENT INSURANCE PLAN

Information is available for the 2007-08 student insurance plan.  Pick up a brochure and application form on the Boca campus at Student Health Services (SS-8W, Room 235), the Immunization Office (SU80, Room 114) or International Students (SU80, Room 214).  Or you can find information by going to www.insuranceforstudents.com.  Please note that the deadline for purchasing spring semester coverage has passed.  If you are interested in purchasing insurance coverage for the summer session, it must be purchased by June 11, 2008.

A representative from Insurance for Students is in the International Students office (SU80, Room 214) on Thursdays from 1:00 p.m. to 4:00 p.m.

2007-08 FAU Student Injury & Sickness Insurance Highlights

• $250,000 maximum lifetime benefit
• $350.00 deductible (waived at FAU Student Health Center)
• 100% coverage at FAU Student Health Center
• $1,000.00 Rx benefit.  Rx co-pay at FAU Pharmacy:  $5 for Tier 1 medications; $15 for Tier 2 and 3 medications.
• Benefits payable at 80% of benefit limits within the UHC Network of providers and 60% outside of the Network
• Nurse triage after hours service (1-800-883-2951).
• Dependent coverage available to enrolled students.

Open enrollment periods:

• Summer                5/12/2008 - 6/11/2008

(Enrollment will not be allowed past the deadline).
